Periodontology
Periodontology
Periodontology is a branch of dentistry that focuses on the study, diagnosis, prevention, and treatment of diseases and conditions affecting the periodontium, the structures surrounding and supporting the teeth. This includes the gums, periodontal ligament, cementum, and alveolar bone.
Stages of Periodontitis:
Gingivitis: The mildest form of periodontal disease, characterized by inflammation of the gums due to plaque buildup. Symptoms include redness, swelling, and bleeding gums. Gingivitis is reversible with proper oral hygiene.
Early Periodontitis: If gingivitis is left untreated, it can progress to early periodontitis, where there is mild bone loss. The gums begin to recede, and periodontal pockets start to form.
Moderate Periodontitis: This stage involves more significant bone loss, deeper periodontal pockets, and potential tooth mobility. Treatment becomes more complex, often requiring surgical intervention.
Advanced Periodontitis: The most severe stage, characterized by extensive bone loss, tooth mobility, and possibly tooth loss. The gums may recede significantly, and pockets can deepen. Treatment often involves surgical procedures and may lead to tooth extraction if the damage is too severe.
Clinical Conditions and Diseases in Periodontology:
Gingivitis: Inflammation of the gums without bone loss.
Periodontitis: Inflammation leading to bone loss and pocket formation.
Aggressive Periodontitis: A rapid form of periodontitis that can occur in otherwise healthy individuals.
Chronic Periodontitis: A slow-progressing form of periodontitis commonly seen in adults.
Necrotizing Periodontal Diseases: Involves tissue necrosis, typically seen in immunocompromised individuals or those with severe malnutrition.
Various Treatment Modalities:
Scaling and Root Planing (SRP): A non-surgical procedure to remove plaque and tartar from below the gumline and smooth the root surfaces.
Periodontal Maintenance: Regular cleaning and examination to prevent recurrence of periodontal disease.
Flap Surgery (Periodontal Surgery): A surgical procedure to access the root surfaces and clean deep periodontal pockets, often involving the lifting of gum tissue to clean and repair underlying structures.
Bone Grafting and Guided Tissue Regeneration: Techniques to rebuild lost bone and encourage tissue regeneration.
Laser Therapy: Use of lasers for various periodontal procedures, such as removing infected tissue, promoting healing, or aiding in depigmentation.
Local Drug Delivery: Applying antimicrobial agents directly into periodontal pockets to target infection and inflammation.
Flap Surgery: Flap surgery involves making incisions in the gum tissue to create a flap, allowing the periodontist to access deeper periodontal pockets for thorough cleaning and debridement. This procedure is used to reduce pocket depths, promote healing, and improve gum tissue attachment to the teeth.
Depigmentation: Depigmentation is a procedure used to remove excess pigmentation from the gums. It can be done using various methods, such as scalpel techniques, chemical treatments, or lasers. This cosmetic procedure helps achieve a more uniform gum color.
Local Drug Delivery Technique: This technique involves placing antimicrobial or antibiotic agents directly into periodontal pockets to reduce bacteria and inflammation. It's typically used in conjunction with other treatments like scaling and root planing to enhance effectiveness.
Use of Lasers in Periodontology: Lasers are used in periodontology for a variety of applications, including:
Laser-Assisted New Attachment Procedure (LANAP): A minimally invasive procedure that uses lasers to target and remove infected tissue without harming healthy tissue.
Laser Gingivectomy: Using lasers to reshape or remove gum tissue for cosmetic or medical reasons.
Laser Depigmentation: Removing excess pigmentation from the gums.
Laser Bacterial Reduction: Reducing bacteria in periodontal pockets to promote healing and reduce inflammation.
These applications of lasers offer benefits like reduced bleeding, quicker healing, and less discomfort compared to traditional methods.
